Medical History

Please know your medical history and a list of current medications.

Current Symptoms

Please jot down your current symptoms or be able to describe them to Dr. Ahumada and her team.

Family History

Please know your family medical history as well as any eye diseases that members of your family have suffered from.

Insurance

Please bring your medical insurance card, ID, and a method of payment if necessary. Contact us ahead of time if you have any questions.

Current Eye Wear

Please bring in your eyewear and/or your most recent prescription for any glasses you currently wear.

Current Contact Lenses

Please bring in your most recent contact lens prescription, pair of new/unopened contacts, or contact lens boxes so that we may know what you are currently wearing.
